Legal Notice
Before starting the intervention, please review the following terms and conditions:
TERMS AND CONDITIONS
Service Eligibility and Area Restrictions
Please note that as I am currently employed by the NHS in the Mid Essex area through Provide CIC, I am unable to offer independent speech and language therapy services to children or families residing in this region. This includes, but is not limited to, the areas of Chelmsford, Braintree, Witham, Maldon, and Halstead. This restriction is in place to prevent any conflict of interest and to ensure full compliance with professional and ethical guidelines. If you are unsure whether this applies to your location, please feel free to contact me for clarification.
Consent to treatment:
By attending the assessment and therapy sessions, you are deemed to have consented to treatment and confirmed that you have read and understood the terms and conditions.
Sharing information with relevant professionals:
It is considered best practice to share information with relevant professionals to ensure your child receives appropriate therapy and that the SLT can work efficiently and effectively with your child.
Speech Spark is dedicated to protecting your child’s privacy. Therefore, your consent will always be obtained before sharing information or reports with other professionals, unless required by law to disclose personal data.
Discharge
At the end of each course of therapy (i.e., a block of sessions), therapy goals will be reviewed to assess whether your child has reached their potential in the targeted area. Based on this review, it may be appropriate to either continue with another block of therapy, take a short break while others implement the speech therapy program with a planned review, or discharge your child from therapy if all goals have been met and they have reached their maximum potential.
You also have the option to withdraw from therapy or postpone it at any time.
Fees
All costs are outlined on the Fees page and are agreed to upon attending the assessment and therapy sessions.
Complaints
If you are not happy with any aspect of the service, please discuss it as soon as possible with your child's therapist. If you continue to have a concern, then please contact the Association for Speech and Language Therapists in Independent Practice to discuss it.
Privacy Statement
This statement sets out the privacy you can expect from Speech Spark in relation to the personal data that is held about your child and the use of that information. Personal data is collected and kept on your child in order that the speech and language therapist who is working with your child can provide the most appropriate and evidence-based intervention possible in order to meet your child’s speech and language therapy needs.
Speech Spark is committed to protecting your privacy and will always involve you in deciding whether reports/information are provided to anyone else, unless obliged by law to disclose personal data.
Confidentiality will be observed and personal data held by Speech Spark will be stored securely in a locked facility or on an access controlled electronic system and will not be available to anyone other than those registered at Speech Spark. Portable electronic devices used to hold data about your child will always be password protected to avoid unintentional access by others.
